Wayne LEADS Form

Click on the link below to access the Wayne LEADS Form.  Keep in mind the only Reader with which the form will work is Adobe.

Once the form is opened...

1. Save it to your own local drive
2. Call it what you want but we recommend using your first name and first initial of your last name with LEADS for easy identification, (example: FirstNameLastInitialLEADS.pdf).
3. Now you can start entering information into the form.   You can save it and reopen later or save it and E-mail as an attachment.   

For those non-represented employees who were using the pilot Performance Planning & Development, Final Reviews are now due; however your Performance planning for FY 2010 should be done on the new Wayne LEADS Form above.
